Telehealth Informed Consent

This Telehealth Informed Consent describes how telehealth services work in connection with Healthy Sprays. Healthy Sprays LLC is a product brand and does not itself provide medical care. Telehealth evaluations and prescribing are provided independently by licensed providers via PrescriberNow, and any medication is dispensed by licensed pharmacies. Please read this consent carefully before requesting an evaluation.

What Telehealth Is

Telehealth (or telemedicine) is the delivery of health care services using electronic communications — such as secure online questionnaires, messaging, and, where required, phone or video — to allow a provider to evaluate you and, when appropriate, provide treatment without an in-person visit. During your evaluation, health information may be exchanged electronically between you and the treating provider.

Independent Providers via PrescriberNow

Your evaluation is performed by an independent, licensed physician or nurse practitioner through the PrescriberNow platform. These providers exercise their own independent clinical judgment. Healthy Sprays does not direct, control, or participate in medical decision-making. Your provider-patient relationship, if any, is with the treating provider — not with Healthy Sprays.

Benefits of Telehealth

• Convenient access to a licensed provider without traveling to an office.
• Faster, more flexible scheduling — often without a live video call.
• The ability to obtain an evaluation and, if appropriate, treatment from home.

Risks & Limitations of Telehealth

• Because there is no in-person physical examination, a provider is relying on the information you provide; incomplete or inaccurate information may affect the evaluation.
• In rare cases, information transmitted may be insufficient (for example, poor image quality or missing history) to allow appropriate decision-making, and the provider may recommend an in-person visit instead.
• As with any medical service, telehealth carries the possibility that a condition could go undetected or that a treatment could have unexpected effects.
• Despite reasonable security measures, electronic transmission of information carries a small risk of technical failure or a breach of privacy.

No Guarantee of a Prescription

Requesting an evaluation or making any payment does not guarantee that you will be diagnosed with any condition or prescribed any medication. The treating provider may determine that treatment — including any Healthy Sprays product — is not appropriate for you. THIN®, THIN-R®, and THIN-ST® are compounded, prescription-only medications and are not FDA-approved; they are dispensed only if a provider determines it is medically appropriate.

Your Right to Withdraw

Your participation in telehealth is voluntary. You may withhold or withdraw your consent to telehealth at any time without affecting your right to future care or treatment, and you may seek care through in-person or alternative means. Withdrawing consent may mean that an evaluation cannot be completed through this program.

Privacy

Information shared during your telehealth evaluation is protected under applicable privacy laws and is handled by PrescriberNow and your treating provider under their own privacy and HIPAA practices. Consent

By requesting a telehealth evaluation, you acknowledge that you have read and understood this consent, that you have had the opportunity to ask questions, and that you voluntarily consent to receive telehealth services from an independent licensed provider via PrescriberNow.
